What is Opthalmic Plastic Surgery ?

Opthalmic Plastic Surgery or 'Oculoplasty'  is the branch of Opthalmology that deals not only with the diseases of the eye ,but also important structures around the eyes like eyelids ,eyebrows ,orbit and the tear system which are vital to the normal appearance and function of our eyes .

Common Conditions Needing Ophthalmic Plastic Surgery 

Opthalmic Plastic Surgery helps correct abnormalities of eyelids like :

(1)Drooping of the lids (Ptosis )

(2) In -turning (Entropion )of lid margin .

(3)Out -turning (Ectropion )of lid margin .

(4 )Lid deformities resulting from trauma.

(5)Problems related to excessive watering from the eyes .



Before and After Ectropion and Entropion Surgery Photos

This patient has a chronic ectropion of her right lower lid. She underwent repair of an eyelid trauma decades ago and now has slowly worsening ectropion which is resulting in ocular irritation, tearing, and redness to the eye. Note that her right lower lid is turned out.
Preoperative diagnosis: Right lower lid ectropion
Procedure performed: Ectropion repair, right lower lid. She underwent a right lower lid ectropion repair and using a special resuspension and tightening technique referred to as a canthoplasty. Her symptoms have resolved and now also has a normal appearing right lower lid.


This patient has a chronic ectropion of his left lower lid. Note that his left lower lid is turned out.. This is more likely due to long-term sun exposure.By repairing this using a special resuspension and tightening technique referred to as a canthoplasty, his tearing complaints improved.
Preoperative diagnosis: Left lower lid ectropion
Procedure performed: Ectropion repair, left lower lid.

This lady had a left lower lid ectropion. As you can see, the left lower lid is lax (or loose) and is turned outward.The left lower lid ectropion has been repaired using a special tightening and resuspension technique.

It is a little subtle at first, but note the inward turning of the right lower lid (entropion). He also has constant irritation and tearing. The right lower lid underwent an entropion repair to help "turn" the lid bac to a more normal position.

Note the inward turning of the eyelashes (entropion) in both lower lids. She also has constant irritation and tearing. Both lids were repaired and turned back to a more normal position. She has ptosis as well but did not want her upper lids repaired.

This patient had trauma to the right lower lid and it was unsuccessfully repaired in the past. She underwent eyelid reconstructive surgery in order to free up and excise the scar tissue and to repair the lower lid position by using a tissue graft along with lid resuspension.
Preoperative Diagnosis: Right lower lid retraction
Procedure performed: Repair of right lower lid retraction using a posterior spacer tissue graft and lid resuspension through canthoplasty.



Preoperative Diagnosis: Right lower lid ectropion and bilateral upper lid ptosis. She did not wish to have upper lid surgery so we only focused on repairing her droopy right lower lid which was resulting in tearing and ocular discomfort.

Preoperative Diagnosis: Left lower lid ectropion. Note that the medial portion (closest to the nose) is everted outward so the puncta (the opening of the tear duct) is not facing the eye. This needs to be corrected otherwise the patient will complain of tearing, discomfort, and may be at risk for an ocular infection.
Procedure performed: Left lower lid ectropion repair. The lower lid was resuspended; in addition, the medial portion of the lower lid (closest to the nose) had an additional procedure to help better invert the puncta (opening of the tear duct) in order to face the eye. The patient had resolution of his tearing symptoms and ocular irritation.

Preoperative diagnosis: Left lower lid ectropion from prior eyelid skin cancer (basal cell carcinoma) resection. The removal of the skin cancer resulted in skin contracture and lid retraction near the eyelid. As a result, the patient complained of a malpositioned lid, tearing, and ocular (eye) irritation. Note that the retraction is worsened when the patient looks up (and with his mouth opened); this suggests a cicatricial (scar or shortening of the thin eyelid skin) component.
In order to appropriately reposition his lower eyelid, a skin graft was required. He had already lost skin from him prior skin cancer resection, and procedures that would not require a skin graft (ie mid face lift with a canthal suspension) may end up working but only a short amount of time. This photo was taken 8 months after undergoing skin graft to resurface the lower eyelid with an ectropion repair. The skin graft was taken from behind his ear (preauricular skin graft).

Procedure Performed: Bilateral lower lid ectropion repair, left lower lid midface resuspension (midface lift). The midface resuspension allows for better support for her lower lid position and to help prevent it from recurring months or years after surgical repair.
